No, theres nothing surgical about it. He is just getting interviewed (not even in english..I think) and the chat show presenter is cracking up at his high voice infront of the stone cold crowd.

Its nowt to do with surgery or someone losing their gonads. Its a clip from a Belgian tv show (kinda like a candid camera thing). Its quite a big show in Belgium and Holland, and is a complete wind up - it wasnt a serious interview.
