I have this local shaping shop who want me to send in a portfolio... one problem I have no clue what goes in it other than the obvious (name, experience, age, pictures and videos.) So any advice would be nice.

The first and most pertinent question would be why are they asking you for a portfolio?
Are you applying for a position, job, to do a photo shoot, that would help us give you answers, do they want to onshore your surfing?

Oh sorry about that they are offering to sponsor me so I can proceed into the ranks of the ASP, in other words I am trying to go pro.

About your portfolio: Be yourself , put down your desires and briefly how you plan to achieve them, what you have to offer in your surfing and personality to your sponsor, include your likes in music, who inspires you both in surfing and in other pursuits, pictures of your early surfing and your contest results, other sponsors if you have any. Outside interests and other occupations you might like to follow.
What do you think you have that would make you likely to succeed amongst the thousands who have this dream.
Form a clear plan of action about how you wish to achieve your goals

Don't BullS...t about yourself but be clear in what you want to do, if it is realistic then you may well do it!
I wish you luck.

And include a couple (not hundreds) of good photos of your surfing. Good luck, and I wish you all the best! It's something I can only dream of.
