Hood, boots, gloves, minimum is a 5/3 but I wouldn't go in without a 6/4 no zip. It is very cold there. I could be wrong but I think the water temp is around 5 deg. C in the winter. And the air temp can be below freezing quite often. I also wished I had earplugs the last time I was there as my ears hurt for 3 days after.
Surf wasn't bad. It can get windy in the winter there, then the waves just get knocked down completely. But if it is sunny and calm, the waves can get quite good. Lots of rental places to get boards. Can get fierce with localism. I think that if you google you can find surf maps of Tofino.
