For some reason, I feel like i'm not getting my back foot as far back on the tail as I like and every now and again my positioning is off. Do you guys slide your back foot as you swing your other foot forward during takeoff, or do you guys just jump up simultaneously?

Are you using a tail pad?
One tip is to leave an area un waxed so that on pop up your back foot will slide into what is deemed to be the natural position.
After a few surfs like this your problems should be ironed out.

how big is the board you're riding? on a longer board you need to make more adjustments in your footing. likewise, the shorter your board, the fewer adjustments you need to make since you'll probably pearl if you're standing tofar up the nose. a deck grip will definitely help in terms of awareness of where you're back foot is. also, look at your foot prints or toe prints on your wax, which is a good indicator of where your foot placement is. in general, you should be popping up on both feet simultaneously. perhaps if you're taking off late or in white water you might be finding it difficult to pop up quickly.
